A certain area or a process gradually fall into disrepair until the upper management takes notice, a Kaizen brigade gets brought in to clean things up, and everything is nice and shiny again. But then, either no or grossly inadequate resources are allocated to the part about sustaining. So things go smoothly for a few weeks or months until neglect begins to accumulate again, with only an occasional fire drill to pretty things up before a bigwig visit.
